KETOS: Clinical decision support and machine learning as a service â A training and deployment platform based on Docker, OMOP-CDM, and FHIR Web Services
Background and objective
To take full advantage of decision support, machine learning, and patient-level prediction models, it is important that models are not only created, but also deployed in a clinical setting. The KETOS platform demonstrated in this work implements a tool for researchers allowing them to perform statistical analyses and deploy resulting models in a secure environment.
Methods
The proposed system uses Docker virtualization to provide researchers with reproducible data analysis and development environments, accessible via Jupyter Notebook, to perform statistical analysis and develop, train and deploy models based on standardized input data. The platform is built in a modular fashion and interfaces with web services using the Health Level 7 (HL7) Fast Healthcare Interoperability Resources (FHIR) standard to access patient data. In our prototypical implementation we use an OMOP common data model (OMOP-CDM) database. The architecture supports the entire research lifecycle from creating a data analysis environment, retrieving data, and training to final deployment in a hospital setting.
Results
We evaluated the platform by establishing and deploying an analysis and end user application for hemoglobin reference intervals within the University Hospital Erlangen. To demonstrate the potential of the system to deploy arbitrary models, we loaded a colorectal cancer dataset into an OMOP database and built machine learning models to predict patient outcomes and made them available via a web service. We demonstrated both the integration with FHIR as well as an example end user application. Finally, we integrated the platform with the open source DataSHIELD architecture to allow for distributed privacy preserving data analysis and training across networks of hospitals.
Conclusion
The KETOS platform takes a novel approach to data analysis, training and deploying decision support models in a hospital or healthcare setting. It does so in a secure and privacy-preserving manner, combining the flexibility of Docker virtualization with the advantages of standardized vocabularies, a widely applied database schema (OMOP-CDM), and a standardized way to exchange medical data (FHIR)

Incident type 2 diabetes attributable to suboptimal diet in 184 countries
The global burden of diet-attributable type 2 diabetes (T2D) is not well established. This risk assessment model estimated T2D incidence among adults attributable to direct and body weight-mediated effects of 11 dietary factors in 184 countries in 1990 and 2018. In 2018, suboptimal intake of these dietary factors was estimated to be attributable to 14.1 million (95% uncertainty interval (UI), 13.8â14.4 million) incident T2D cases, representing 70.3% (68.8â71.8%) of new cases globally. Largest T2D burdens were attributable to insufficient whole-grain intake (26.1% (25.0â27.1%)), excess refined rice and wheat intake (24.6% (22.3â27.2%)) and excess processed meat intake (20.3% (18.3â23.5%)). Across regions, highest proportional burdens were in central and eastern Europe and central Asia (85.6% (83.4â87.7%)) and Latin America and the Caribbean (81.8% (80.1â83.4%)); and lowest proportional burdens were in South Asia (55.4% (52.1â60.7%)). Proportions of diet-attributable T2D were generally larger in men than in women and were inversely correlated with age. Diet-attributable T2D was generally larger among urban versus rural residents and higher versus lower educated individuals, except in high-income countries, central and eastern Europe and central Asia, where burdens were larger in rural residents and in lower educated individuals. Compared with 1990, global diet-attributable T2D increased by 2.6 absolute percentage points (8.6 million more cases) in 2018, with variation in these trends by world region and dietary factor. Childrenâs and adolescentsâ rising animal-source food intakes in 1990â2018 were impacted by age, region, parental education and urbanicity
Animal-source foods (ASF) provide nutrition for children and adolescentsâ physical and cognitive development. Here, we use data from the Global Dietary Database and Bayesian hierarchical models to quantify global, regional and national ASF intakes between 1990 and 2018 by age group across 185 countries, representing 93% of the worldâs child population. Mean ASF intake was 1.9 servings per day, representing 16% of children consuming at least three daily servings. Intake was similar between boys and girls, but higher among urban children with educated parents. Consumption varied by age from 0.6 at <1 year to 2.5 servings per day at 15â19 years. Between 1990 and 2018, mean ASF intake increased by 0.5 servings per week, with increases in all regions except sub-Saharan Africa. In 2018, total ASF consumption was highest in Russia, Brazil, Mexico and Turkey, and lowest in Uganda, India, Kenya and Bangladesh. Strome: Energy-Aware Data-Stream Processing
International audienceHandling workloads generated by a large number of users, data-streamâprocessing systems also require large amounts of energy. To reduce their energy footprint, such systems typically rely on the operating systems of their servers to adjust processor speeds depending on the current workload by performing dynamic voltage and frequency scaling (DVFS). In this paper, we show that, although effective, this approach still leaves room for significant energy savings due to DVFS making conservative assumptions regarding its impact on application performance. To leverage the unused potential we present Strome, an energy-aware technique to minimize energy demand in data-streamâprocessing systems by dynamically adapting upper limits for the power demand of hardware components. In contrast to DVFS, Strome exploits information on application performance and is therefore able to achieve energy savings while minimizing its effects on throughput and latency. Our evaluation shows that Strome is particularly effective in the face of varying workloads, reducing power demand by up to 25 % compared with the state-of-the-art data-streamâprocessing system Heron relying on DVFS
Prototypical clinical trial registry based on fast healthcare interoperability resources (FHIR): design and implementation study
Background: Clinical trial registries increase transparency in medical research by making information and results of planned, ongoing, and completed studies publicly available. However, the registration of clinical trials remains a time-consuming manual task complicated by the fact that the same studies often need to be registered in different registries with different data entry requirements and interfaces.
Objective: This study investigates how Health Level 7 (HL7) Fast Healthcare Interoperability Resources (FHIR) may be used as a standardized format for exchanging and storing clinical trial records.
Methods: We designed and prototypically implemented an open-source central trial registry containing records from university hospitals, which are automatically exported and updated by local study management systems.
Results: We provided an architecture and implementation of a multisite clinical trials registry based on HL7 FHIR as a data storage and exchange format.
Conclusions: The results show that FHIR resources establish a harmonized view of study information from heterogeneous sources by enabling automated data exchange between trial centers and central study registries
Patient Recruitment System for Clinical Trials: Mixed Methods Study About Requirements at Ten University Hospitals
BackgroundClinical trials are the gold standard for advancing medical knowledge and improving patient outcomes. For their success, an appropriately sized cohort is required. However, patient recruitment remains one of the most challenging aspects of clinical trials. Information technology (IT) support systemsâfor instance, patient recruitment systemsâmay help overcome existing challenges and improve recruitment rates, when customized to the user needs and environment.
ObjectiveThe goal of our study is to describe the status quo of patient recruitment processes and to identify user requirements for the development of a patient recruitment system.
MethodsWe conducted a web-based survey with 56 participants as well as semistructured interviews with 33 participants from 10 German university hospitals.
ResultsWe here report the recruitment procedures and challenges of 10 university hospitals. The recruitment process was influenced by diverse factors such as the ward, use of software, and the study inclusion criteria. Overall, clinical staff seemed more involved in patient identification, while the research staff focused on screening tasks. Ad hoc and planned screenings were common. Identifying eligible patients was still associated with significant manual efforts. The recruitment staff used Microsoft Office suite because tailored software were not available. To implement such software, data from disparate sources will need to be made available. We discussed concrete technical challenges concerning patient recruitment systems, including requirements for features, data, infrastructure, and workflow integration, and we contributed to the support of developing a successful system.
ConclusionsIdentifying eligible patients is still associated with significant manual efforts. To fully make use of the high potential of IT in patient recruitment, many technical and process challenges have to be solved first. We contribute and discuss concrete technical challenges for patient recruitment systems, including requirements for features, data, infrastructure, and workflow integration
Canola growth, production and quality are influenced by seed size and seeding rate
Canola (Brassica napus L.) is the most widespread profitable cash crop in Canada. In 2014 and 2015, direct-seeded experiments were conducted in sixteen western Canada environments. âSmallâ canola seed (average 3.32 to 3.44 g 1000-1) was compared to âlargeâ canola seed (average 4.96 to 5.40 g 1000-1) at five seeding rates (50, 75,100, 125 or 150 seeds m-2). Large canola seeds increased crop density and crop biomass but decreased plant mortality, days to start of flowering, days to end of flowering, days to maturity and percent green seed. Seed size did not influence harvested seed weight, seed oil content or seed protein content. Increasing the seeding rate of small seeds improved canola yield, but the same response did not occur for large seeds. Increasing seeding rates also increased crop density, plant mortality, crop biomass and seed oil content but decreased days to start of flowering, days to end of flowering, days to maturity, percent green seed and seed protein content. Seeding rate had no impact on harvested seed weights.
